This notice explains the information collected through this website’s plan-review request form and how it is used.

Information you choose to share

The form asks for your name, business email, company, role, approximate plan size, review priority, and permission to respond. Additional notes are optional. A request reference and receipt time are also recorded.

Please do not include Social Security numbers, participant records, account credentials, or other sensitive personal or financial information. If a later review needs additional documents, an appropriate process should be agreed separately.

How your request is used

Your information is used to assess and respond to your inquiry and coordinate the next conversation about your plan. Submitting the form does not sign you up for unrelated marketing or commit you to an advisory engagement.

Storage and access

Requests are stored using Cloudflare infrastructure. Access to request records is restricted to authorized account users. Information may be shared with service providers needed to operate the website and handle the inquiry, or where required by law.

Information should be kept only as long as needed for the inquiry, any ensuing relationship, and applicable recordkeeping requirements. Do not use this website to send documents intended for secure client storage.

Cookies and the cost calculator

This website does not use advertising trackers or third-party analytics. Calculator inputs are processed in your browser. If you choose “Discuss your figures,” those figures may be added to your request notes; they are sent only when you submit the form.

Hosting providers may process technical information needed to deliver and protect the website. If you follow an external scheduling link, that provider’s privacy terms apply to the information you enter there.

Questions about your information

You can use the request form to ask about an earlier inquiry or request a correction or deletion, subject to any applicable recordkeeping requirements. Include your request reference if you have it, and avoid adding sensitive information.
